She is CERF normal, OFA excellent, Brucellosis negative, DNA-VIP and current on all vaccinations. To their knowledge, the Breeders certify that the dog is seizure free. (contain other relevant info about the dog like purebred or cross status). All puppies produced will be placed in approved homes under contract.

(name) is a one-year old male of sound temperament and good physical condition. He is CERF normal, GDC Good, Brucellosis negative and current on all vaccinations. To their knowledge, the Owners certify that the dog is seizure free. (contain other relevant info about the dog like purebred or cross status and if the male is a proven stud).

Owners agree to breed (name of sire) to (name of dam). Stud service is due at a mutually agreed upon time and place. Owners agree to allow (name of sire) to be drawn for artificial insemination if natural breeding does not occur. However, Owners do not authorize the Breeders to keep, store or otherwise use the semen for any other purpose. Owners agree to collect a DNA sample from (name of sire) prior to or at the time of breeding and submit the specimen to the UKC for DNA profiling. Owners understand that all puppies from the litter will be DNA verified identified parentage.

Owners agree to provide Breeders with copies of (name of sire) pedigree, GDC certificate, CERF certificate and two recent photographs suitable for use in advertising the litter. Breeders will provide copies of CERF and OFA certificates of (name of dam) to Owners.

If the breeding does not result in pregnancy or, if DNA tests conclude that no puppies resulted from this breeding, no valuable consideration shall pass from Breeders to Owners. DNA reports will be provided to the Owners to confirm parentage. If the dam conceives and delivers at least one live puppy, who lives to two weeks, and that puppy is determined to be the offspring of the stud, stud fee will be paid to the Owners as outlined below. If the dam whelps at least one puppy, but does not whelp three puppies, live at two weeks, from her breeding with the sire, then a free stud service will be provided to Breeders at a mutually agreeable time in the future (to a female of the Breeder’s choice)(the same female). All transportation charges and veterinary expenses, including any costs for artificial insemination if natural breeding does not occur, will be the responsibility of the Breeders.

Stud Fee: The Owners may elect one of the following two options if stud fee is to be paid. The Owners may either receive:

1. $600.00 paid to the Owners upon receipt of DNA test results confirming parentage; or

2. A "pet quality" puppy from this breeding. If the Owners elect to receive a puppy, they must make the election within a week of the birth of the puppies. The Breeders will make the determination of which puppies are "pet quality". If more than one puppy is determined to be "pet quality", Owners will be given "first pick" from the available "pet quality" puppies. Owners must make this pick by the time the puppies are five weeks old or will waive the right to make the pick.

(The option could also be a pick breeding puppy).

All parties certify that they are in good standing with the UKC.

This is a legally binding contract controlled by the laws of (state of breeders). It is agreed that the place of venue shall be in (state of breeders). This contract contains the entire agreement of Owners and Breeders. Oral modifications shall not be permitted. In the event that the parties agree to additional terms, these terms shall be reduced to writing and attached as an addendum hereto. If any provision of this agreement is held to be invalid, such invalidity shall not affect other provisions hereof.

By signing this contract, the Owners and Breeders certify that they have the consent of other individuals who are not parties to this contract to breed the dog as required by any ownership contracts.
